It was not overly uncommon to hear the name in 1970 or '71 in the UK or Australia - I think a considerable number of parents selected the name after hearing it for the first time on an English BBC (ABC in Australia) wildlife programme starring the couple MICHAELA and ARMAND DENIS. Perhaps this is also true of other Commonwealth nations where the programme aired. I've heard pronunications vary from mi-kay-la to mi-ky-la to mi-kar-la (for those born in the'70s) but the mainstream pronunciation today seems to be mi-kay-la, with phonetic spellings gaining popularity."
